for the code of the radiological procedure that has been requested.
If you are unable to obtain the |

procedure
code, get as much information on the procedure as possible so we can
give you accurate information. |
3.
Who is ultimately responsible for the bill?
 |
We will bill
your insurance provider if you provide our office with the correct
information. However, if your insurance |
 |
provider does
not pay your bill, you will be responsible for contacting them to ensure
payment of your account. |

Ultimately, you -
the patient - are responsible for the bill. We get your information directly
from the hospital when |
 |
you register, but there
are times that the information sent to us is incorrect and it is your
responsibility to make |
 |
sure that we have the
most up to date information to get your bill paid. |
4. How
often will I be mailed a statement?
 |
If you have
insurance, you will be mailed a statement after the insurance has sent us a
remittance advice telling us |
 |
how much they have paid
and adjusted your bill. Then you will receive a statement every 30-45 days. We
send |
 |
delinquent accounts to
collections on a regular basis after 90-120 days. |
5. After
how much time should I contact my insurance plan if no payment has been made/
 |
Usually if your
insurance company has not processed your bill within 45-60 days, we
recommend that you get in |
 |
touch with your provider
to determine the status of your account. |
6. Why will
I receive two bills for my procedure?
 |
Radiology procedure
charges are billed in two parts: the hospital and the physician who
interprets the examination. |
 |
The radiologist directs
and supervises your examination, interprets the results and consults with your
personal |
 |
physician. You may have
met the radiologist personally during the exam, but even if you did not, he or
she will be |
 |
studying your
examination and sending reports to your personal physician. The bill is your
responsibility. Any |
 |
insurance is between the
patient and the insurance carrier. The hospital portion of the bill covers the
technicians, |
 |
equipment and supplies
involved in performing the procedure. |
